It's in the cgi-bin directory on creosote.  I modified it for the SpamBayes
project and have it installed on the Musi-Cal staging server for the
moment.  You can use it here:

    http://staging.musi-cal.com/cgi-bin/sf

It has the simple form modification.  I've attached the code.
